If you are using 64 Bit windows, I think I may know the answer is. But in general you need to ensure the icon is associated with the link in the Windows menu. So
The icon should be present (blue circle)
While you could enter icon location here (for windows 7 - 64 bit it is probably C:\Program Files (x86)\RecordEdit\MSaccess\lib\RecordEdit.ico for the MsAccess version) it would be better to click on the browse button.
